Where the per-user model bites hardest

Skilled home health runs on a clinician roster that changes. Under per-user pricing that produces two costs: you keep paying for accounts belonging to clinicians who left until someone deprovisions them, and you pay again when you hire the replacement.

It also produces a quieter cost. When a login has a price attached, small agencies ration them, and shared accounts appear. At that point the audit trail stops being attributable to an individual — which is the entire reason a surveyor wants an electronic record in the first place.
